Space Closure – it is a matter of perfect timing

Several surgical procedures have been advanced in a high-tariff quest to accelerate orthodontic treatment along with regional accelerated phenomenon. These procedures shall range from minimally invasive surgery to minimal insult. Unfortunately, studies have shown that such procedures end up in lukewarm results. It also seems that repeated intervention may be necessary to produce sustained surgical acceleration – a prospect that might not appeal to patients or providers and can also have a biological cost.

We all might know that tooth extractions are conducted to offer the necessary space to achieve orthodontic treatment objectives. But when should we commence this space closure? Should we start extracting immediately before initiating a space closure to avail of a potential RAP and accelerate the space closure phase? OR should we extract it a little earlier, permitting the area to get matured and organised, defer space closure and use the space.

There was a study conducted on participants of 15-29 years, let us now see what happened to them.

These participants were to remove their first premolars as part of their orthodontic treatment.

Immediately after the first alignment and engagement of wire, the doctors extracted the maxillary first premolar randomly on one side. Later they removed the contra lateral first bicuspid 1 month later. Right after 8 days, the molars were stabilised, and the retraction of individual canines started. The rate of the canine retraction was 1 month.

This study threw light on the important question of accelerating tooth movement. Although, this does not involve any additional intervention. It was also the first randomised controlled trial on this topic. It also addressed the glaring gap in dental literature, the write-up was quite clean and methodology was sound as well.

They observed differences in the canine retraction in two different groups and this was a significant achievement.
