Forms are the first thing you need to do.

Notice: Verified doctors may now submit electronic physician certification forms for patients who qualify. To get started, patients must have a Missouri-licensed MD or DO fill out the Physician Certification Form.

The Standard Physician Certification Form may be used by individuals asking for the standard recommended dosage of four (4) ounces per month.

It is necessary for patients who need more than the customary four ounces of medication to fill out an Alternative Physician Certification Form. This form must be completed by two doctors who must both check the same qualifying medical condition. If the doctors' suggested quantities on the paperwork are conflicting, the patient will be granted the smaller quantity.

You will also need to submit a Parent/Legal Guardian Consent Form for patients under the age of 18.

A Patient Authorization Form and the patient's license number, which they will get after their application is granted, are required for the application of carers. PAT will be the first digit of this number.

Step 2: Fill out an application for membership.

Patients must first create an account on the Complia application site in order to proceed with the application process.

For further information and to submit an application for an identity card, click on the blue Patient/Caregiver Application Registration box under your picture. To access the registry, click "Yes" in the "Warning-Exiting Site" pop-up box. On-line applications will be the only ones that will be accepted.

Application is the third step.

By login into the Complia site, clicking "+Create Application" and "New Patient Registration", an application may be completed.

Once a caregiver has signed up, an application may be completed by login into the Complia site and selecting "+Create Application" and "New Caregiver Registration".

There are 30 days to process all applications, and they are processed in the order that they were received by the department.
