The mechanism of action of Drug Eluting Balloon involves three key steps. First, the balloon is inflated at the site of the lesion, resulting in mechanical vessel dilatation. This process disrupts the plaque and creates microdissections in the vessel wall, enhancing drug penetration. Second, the antiproliferative drug is released from the balloon's coating, diffusing into the surrounding tissue. The drug then exerts its therapeutic effect by inhibiting smooth muscle cell proliferation and reducing neointimal hyperplasia. Finally, the balloon is deflated and withdrawn, leaving behind a vessel that is free from both the underlying disease and any permanent implants.


Numerous clinical trials and real-world studies have demonstrated the efficacy of drug-eluting balloons in various cardiovascular conditions. In coronary artery disease, DEBs have been shown to effectively reduce restenosis rates compared to conventional balloon angioplasty. Furthermore, Drug Eluting Balloon have proven particularly beneficial in the treatment of in-stent restenosis, a challenging condition characterized by the recurrence of stenosis within previously stented vessels.


Elimination of Permanent Implants: Unlike drug-eluting stents, DEBs do not leave a permanent implant in the treated blood vessel. This can be advantageous in scenarios where long-term stent placement may not be ideal or necessary.

 

Reduced Risk of Stent Thrombosis: Drug Eluting Balloon can be particularly beneficial in cases where the patient may have an increased risk of stent thrombosis due to factors such as bleeding disorders or the need for future surgical interventions. By avoiding the use of stents, the risk of stent-related complications is minimized.

 

Preservation of Future Treatment Options: DEBs allow for the preservation of future treatment options by avoiding the limitations associated with stents. The absence of a permanent implant facilitates potential revascularization procedures, including additional balloon angioplasty, bypass surgery, or stent placement if required in the future.
