The posteromedial wall of the maxillary sinus can be removed if necessary, and then the pterygoid fossa is fully exposed. The vidian nerve or sphenopalatine ganglion in the pterygoid fossa can also be approached with this technique. The petrous apex can be approached with a paraseptal approach. This endoscopic endonasal approach is particularly useful for drainage of cholesterol granulomas.
